APPLICATION FOR MEMBERSHIP

The Pickleball Club’s year of operation is from January 1 to December 31.  Annual dues are $26.00 per person from January 1 to June 30.  New members who join the club anytime from July 1 through the end of the year shall pay $13.00 for the remainder of the year.  By return of the information below, I/we certify that I/we have read the By-Laws and Rules and Regulations and agree to abide with them and any other policies and procedures directed by the board of the Pickleball Club, as posted on the Club’s bulletin board.
 
All applicants must show CA membership cards to a member of the Pickleball Club Board of Directors before submitting an application.  Ask a Board Member to initial or sign below, and then mail or give completed application and your check  Sam Montana, Treasurer, 2009 Sterling Glenn Ct.  SCC, FL. 33573.
